(1) If you are newly eligible for Medicare due to disability or turning 65, you can apply online at www.ssa.gov/locator
(2) Whether you are applying for Retirement Benefits with Medicare or Medicare alone, you can do so at “Apply for Retirement Benefits.” The application is the same link.

(3) If you are applying for Part B only or losing employer group coverage, you will need to apply by mail. The links below will provide the Part B forms that would need to get completed and mailed to the client’s local SSA office. DOWNLOAD THE FORMS BELOW:
